2.48.100 Reports to commission.
For the purpose of certification to the city officers for the payment of salaries and in order that the commission may keep a proper record of changes in the classified civil service, each appointing officer shall immediately report in writing to the commission prior to the next regular meeting.
A. Every probational or temporary appointment made;
B. Every refusal or neglect to accept an appointment under the department head by a person who has been duly certified;
C. Every suspension or other disciplinary action made by the department head of any officer or employee with the reasons thereof;
D. Every separation from service under the department head and whether the separation was caused by death, dismissal or resignation; any such resignation to be in writing and filed with the commission;
E. Every office created or abolished in the department, which comes under the classified civil service;
F. Every leave of absence granted to a member of the department.
