All proceedings conducted pursuant to this chapter shall be commenced in the manner provided by the charter, statute or ordinance governing the matter. An appeal must be in writing and shall contain the following:

A. A brief statement as to how the appellant is significantly affected by or interested in the appeal.

B. A brief statement containing specific exceptions and objections to the decision being appealed.

C. The requested relief, such as reversal or modification.

D. The signature, mailing address, and telephone number of the appellant. A specific agent shall be designated if the appellant is not an individual.

E. Any other information required by the ordinance governing the matter.
