The notice and order shall contain the following:

A. The notice shall be written except as otherwise provided in Section 15.36.110;

B. The street address and a legal description sufficient for identification of the premises upon which the building, structure, or hazard is located;

C. A statement that the electrical inspector has found a hazardous condition with a brief and concise description of the conditions found to render the determination;

D. A statement of action required to be taken as determined by the electrical inspector;

E. A statement of action that has been or will be taken if the hazardous condition is not removed; and

F. A statement advising that any person having any record title or legal interest in the building or structure may appeal from the notice and order any action of the electrical inspector to the board of appeals; provided, the appeal is made in writing and filed with the electrical inspector within thirty (30) days from the date of service of such notice and order; and that failure to appeal will constitute a waiver of all right to an administrative hearing and determination of the matter.
