A notice and order to abate shall contain a description of what constitutes the nuisance, the location of the nuisance, a statement of the acts necessary to abate the nuisance, a reasonable time within which to complete the abatement, a statement that if the nuisance is not abated as directed and no request for hearing is made within the prescribed time or if the responsible person fails to appear at a hearing as set by the enforcement officer, the city will abate it and assess costs against the property owner or other responsible person. A notice and order shall be substantially in the following form:
